 Q1. UCI 在簽證資料上可以找到。

 Q2. 該格Guide有備註留白，除非你有號碼。Leave this section blank. You will only be able to complete this question if you have already been informed of your application number

Personal Details 

 Q1. 英文姓名記得和護照上的拼法一樣。

 Q3. 父母親的英文姓名也和護照上一致。不適用的格子就印出來後手寫 N/A。

 Q4. 就按照個人情況來回答。

 Q5. 請從最近的學歷開始回填到國中學歷。

• Level of Education：國中 Junior high school，高中 Senior high school。
• Field of Study：高職就填入什麼科系，普通科所以填入 General。
• Name of school/institution：Google搜尋「學校名字＋英文」都可以找得到。
• Certificate & Diploma issued：國高中的畢業證書都是 Diploma。

 Q6. 請參考 Guide 所提供的範例，並且從最近開始往回填過去十年。我個人是有兩項活動寫在同一個月份。例如：2010-03 to 2010-05 Unemployed，2010-05 to 2011-05 Saleperson。只要最後可以 Validate 都沒有問題，但有時候會發生明明月份都沒有重疊到但是還是出現 Error，這時候你得把所有工作經驗都清除並重新輸入。

* 通常觀光不到一兩個月以上的所以就不用特別填入。

• Activity Type：就依照當時是Employment, Unemployed, Educational Activity (大部分都是這三種) 做選擇。

• Status in Country：在該國家是持有什麼簽證。

• Name of company, employer, school, facility, as applicable：就上網查查看該公司(學校)英文名稱是甚麼並填入。如果都沒有的話就填入 Not Applicable。

 Q7, 8, 9 依照個人情況回答

我當了四個月的義務兵，所以也必須填入表中。比較麻煩的就是要找出 Name of company 以及 Commanding officer name，我也事先將退伍令拿去給人翻譯已備如果被要求補件。

• Military rank：二兵，Private。

• Dates and places of any active combat：應該都沒有，所以 Not Applicable。

• Reason for end of service：退伍，Completed service。

 Q10. 從最近到過去十年，請不要將住址簡寫，請將住址詳細填入，Rd. 請打成 Road，St. 打成 Street，Dist. 打成 District。Example: 999 Family Street, Unit #3, Ottawa, Ontario, Canada, K3J 9T5

最後輸入電子簽名以及日期，印下來後手寫簽名。

1. In the signature section, type the name of the person for whom the form is being filled out. Also, enter the date that the form is being filled out.
2. Validate the form.
3. Print the form.
4. Sign your name by hand. (Note: For principal applicants under the age of 18, a parent or legal guardian must sign the form on the dependent child’s behalf.)
